Building Bridges Beyond Hospital to Better Health.

At Sentinel Health Partners, LLC, we believe that healthcare does not end when a patient leaves the hospital. Recovery is a journey that continues beyond discharge, requiring coordinated clinical support, compassionate leadership, and strong community partnerships.

Founded on the principles of excellence, stewardship, and service, Sentinel Health Partners was created to strengthen the bridge between healthcare systems and the communities they serve. As a healthcare holding company, we develop and support innovative organizations dedicated to improving care transitions, advancing nursing leadership, expanding access to healthcare services, and creating sustainable solutions that improve lives.

“Every initiative undertaken by Sentinel shall clearly define the problem it seeks to solve, the solution it proposes, the outcomes it intends to achieve, and the way in which it strengthens the bridge between healthcare and sustained well-being.”

Leadership Rooted in Service & Governance

25+ Years of Healthcare Leadership & Strategic Governance

Dr. Nkechi Osuji, DNP, RN, is a seasoned nursing executive and healthcare strategist with more than 25 years of progressive leadership experience across medical and behavioral health, intellectual and developmental disabilities (I/DD), integrated healthcare, and community-based care. As a leader in medical respite programming and fractional Chief Nursing Officer services, Nkechi brings a rare combination of executive vision, clinical expertise, and compassion for individuals navigating complex health and social needs.

Nkechi is known for helping organizations move from vision to operational excellence. She has built sustainable nursing infrastructures, strengthened staffing models, developed quality improvement initiatives, and supported regulatory readiness in settings where safety, dignity, and continuity of care are essential. Her work is especially aligned with medical respite care, where thoughtful clinical leadership can help bridge the gap between hospital discharge, recovery, housing instability, and long-term wellness.

Throughout her career, Nkechi has served in senior leadership roles including Chief Nursing Officer, Senior Vice President of Nursing, Senior Director of Nursing, and Director of Nursing Services in both inpatient and outpatient healthcare environments. She brings a steady, solutions-focused leadership style to complex organizations, supporting nursing teams, advancing patient safety, strengthening clinical governance, and fostering collaboration across clinical, operational, and administrative partners.

Through fractional Chief Nursing Officer services, Nkechi partners with healthcare organizations that need experienced executive nursing leadership without the commitment of a full-time role. She provides strategic oversight, hands-on guidance, and practical systems support in areas such as nursing operations, workforce planning, quality improvement, compliance, care coordination, policy development, and team development.

Nkechi earned her Doctor of Nursing Practice (DNP), Master of Science in Nursing (MSN) from the University of Phoenix, and a Bachelor of Science in Nursing (BSN). She also holds an LPN diploma, is a Certified Case Manager (CCM), and maintains current licensure as both a Registered Nurse (RN) and Licensed Practical Nurse (LPN) in New York State.
